Blake Burchett, MD
Dr. Blake Burchett graduated from the University of Kentucky College of Medicine in 1983. He was previously a former Chief Resident at Spartanburg Medical Center. He began practicing in 1986 and is AAFP certified. He is married to Rebecca Burchett and together they have three beautiful children, Dr. Andrew Burchett of Prestonsburg, KY, Dr. Molly Burchett of Laremy, WY, and Michael Burchett of Lexington, KY. Dr Burchett is currently accepting new patients.
Tasha Russell, DO
Dr. Tasha Russell resides in Salyersville, Ky. Her hobbies include reading, traveling the world on cruises, and spending time outdoors with my family. She is married to Brandon Russell, and they have two wonderful children Brendan and Kenzie, several pets including cats, dogs, chickens and ducks. Her medical interests include educating families about the importance of immunizations and helping mothers and babies through the challenges of breastfeeding.

Nickie is a proud resident of Salyersville, Kentucky. She holds a Bachelor's Degree in Biomedical Science from Morehead State University and a Master's Degree in Physician Assistant studies from the University of Kentucky.
Nickie enjoys an active lifestyle and spends her free time cycling, hiking, kayaking, and mountain biking. She is happily married to Chris, and together they are the proud parents of two children, Riley and Reed.
An active member of Salyersville First Baptist Church, Nickie is dedicated to both her faith and her community.

Trevor Howard PMHNP-BC
Trevor is a board-certified Psychiatric Mental Health Nurse Practitioner. He graduated from Morehead State University with his Bachelor of Science in Nursing in 2016. Since that time, he has served as an emergency department RN in local hospitals. It was with this experience that he noticed there was a great need to improve access to mental health services in our region as mental health is a prevalent problem and is under treated. Trevor wanted to play a part in improving access to these much-needed services and sought to earn his Master of Science in Nursing with a concentration in Rural Psychiatric Mental Health. He graduated from Eastern Kentucky University in May 2022 and became board-certified by the ANCC to treat patients across the lifespan. Trevor began working Frontier Medical in September 2022 and together we have since continued to grow and expand these services to people throughout our region. He believes in creating an individualized plan of care that values client involvement, interprofessional collaboration and evidenced-based practice. His goal for his clients is to feel empowered to treat their illness through supportive education in a stigma free environment.

Amber Koch, LPCA
Amber is a Licensed Professional Counselor. She began her undergraduate career in 2008 at Alice Lloyd College with completion in the spring of 2012 with a Bachelor of Science in Social Science. While working with the underprivileged youth of Eastern Kentucky, she began the master’s program with Walden University where she completed her Master's of Science in Marriage, Couple, and Family Counseling with an emphasis on child and adolescent trauma in 2017. With over 13 years of experience, Amber has continued to work on furthering her education with certifications and skills in evidence-based practices, play therapy, complex trauma certification, and recently finished her certification as an autism clinical specialist. Amber is proud to be a voice for our youth who often are silenced and her goal in office is simple; "I want you to be safe, happy, and to always feel loved."
With her roots being deeply rooted in the Eastern Kentucky mountains, " I feel that it is part of my responsibility to help our youth be a light for the future and if I am not supplying them with motivation for change and perseverance for their future, then I’m not doing my job correctly."
In her free time, you’ll find her on her back porch watching her two Aussie-Doodles, curled up on the couch watching Gilmore Girls, spending time on a sideline basketball court, or sitting near 3rd base with her Husband and her dearest friends, watching her two boys (and the rest of Magoffin County) play until their hearts are content.
Carlee Salisbury, MSN, APRN, FNP-C
Carlee Salisbury, MSN, APRN, FNP-C, is a dedicated Family Nurse Practitioner committed to providing compassionate, patient-centered care across the lifespan. With a strong clinical foundation in obstetrics, she has extensive experience caring for mothers and newborns, she has special emphasis on supporting breastfeeding, postpartum wellness and pediatrics. Carlee is beyond excited to care for your entire family, no matter the age. She earned her Master of Science in Nursing from Northern Kentucky University and is board-certified as a Family Nurse Practitioner. Her background in women’s and newborn health has shaped her holistic approach to care, allowing her to confidently support patients through every stage of life ranging from pediatric care to chronic disease management and preventive health. She is passionate about empowering women and families through education, encouragement, and individualized care plans.
Carlee is especially committed to promoting successful breastfeeding experiences, recognizing its vital role in both maternal and infant health, she has been a certified lactation counselor for eight years. Known for her approachable demeanor and attention to detail, Carlee strives to build meaningful relationships with her patients while maintaining the highest standards of clinical excellence. She stays current with the latest advancements in healthcare to ensure her patients receive the most effective, evidence-based care. Outside of her clinical work, Carlee values church, family, community, and maintaining a balanced, healthy lifestyle.
