Blake Burchett M.D.
Dr. Blake Burchett graduated from the University of Kentucky College of Medicine in 1983. He was previously a former Chief Resident at Spartanburg Medical Center. He began practicing in 1986 and is AAFP certified. He is married to Rebecca Burchett and together they have three beautiful children, Dr. Andrew Burchett of Prestonsburg, KY, Dr. Molly Burchett of Laremy, WY, and Michael Burchett of Lexington, KY. Dr Burchett is currently accepting new patients.
Mary Lez Perry, MSN, APRN, FNP-C
Mary Lez Perry, MSN, APRN, FNP-C, is a dedicated healthcare professional known for her commitment to compassionate patient care and clinical excellence. She earned her Bachelor of Science in Nursing in the Philippines from 2010 to 2014, where she built a strong foundation for her nursing career.
After moving to the United States, Mary began working as a staff nurse at ARH Hazard, gaining valuable clinical experience while caring for diverse patient populations. She later transitioned into travel nursing, expanding her clinical expertise while caring for patients across various healthcare settings.
Mary’s dedication to nursing and her patients has been recognized through prestigious honors, including the 2023 DAISY Award at Pikeville Medical Center, which celebrates extraordinary nurses who demonstrate compassion and excellence in patient care. She was the very first agency nurse at the facility ever to receive this award, highlighting her exceptional commitment to her patients and profession.
While actively working as a nurse, Mary pursued advanced education and completed her Master of Science in Nursing (MSN) from 2019 to 2021. In 2022, she achieved another milestone by becoming a board-certified Family Nurse Practitioner (FNP-C).
Today, Mary serves her community as an Advanced Practice Registered Nurse, combining compassion, clinical experience, and dedication to improving patient outcomes and delivering quality healthcare.
Elisha Ward, APRN
Elisha specializes in practicing Dermatology and has been practicing for 5 years. Her education includes Board Certification for the American Nurses Credentialing Center (ANCC) and her master's in nursing from Chamberlain University. She is a member of the National Academy of Dermatology and the Kentucky Dermatology NP/PA Association. Elisha was a hospitalist at Pikeville Medical Center before joining Frontier Medical. Her passion for giving quality care to each patient with dermatology needs is compelling and heartfelt with not only staff but patients alike. Ensuring I spend the proper amount of time with each and every patient. I am very thorough in my assessment and making a correct diagnosis to be able to treat the patient. Elisha is currently accepting new patients.Â
